We are seeking a talented Hospice Clinical Liaison (RN) for 40 hours per week.

Position Summary:

The Hospice Clinical Liaison (RN), will act as liaison between the contracted hospitals and the hospice team, make inpatient visits to hospice patients in contracted facilities, act as consultant for pain and symptom management, provide in-service education to providers, perform pre-admission evaluations for appropriateness for hospice services, initiates the admission process by educating and informing patient/family of hospice and it’s services. Also obtain informed consents. Provide nursing assessment, initial plan of care and intervention(s), which optimize patient and family directed comfort and quality of life and coordinates discharge planning.

Responsibilities:

  • Provides admission assessment and engages with patient and family to develop rapport and a sense of trust. During the initial visit, performs appropriately timed (as determined by the Management team) physical assessment. Establishes hospice plan of care that includes appropriate Physician orders for pain and symptom management.
  • Completes the Agreement for Time Limited Services form. Provides a clear explanation that this is limited stay for symptom management.  Communicates with Providers Social Worker (SW) to initiate a plan for discharge once the symptoms are managed.  Writes new orders. 
  • If patient is to be admitted or transferred to home or a facility, coordinates admission time with Access Department. Arranges for DME to be delivered through the Access Department.  Communicates with SW or Discharge Planner to coordinate ambulance transport if necessary. Leaves a communication note on the chart in the progress notes requesting appropriate medications to be written for discharge.  Also be sure MEMS form is in chart for Physician signature.  Check the patient’s chart on morning of discharge to be sure that these items are written and in chart. 
  • Conducts death visit and assists in the assessment of family needs for bereavement services following the patient's death and conveys information to bereavement staff no later than three days after death of patient. Notifies attending physician of death.  Completes death packet and submits in a timely manner.
  • Communicates relevant information to the provider, including any change in referral diagnosis.
  • Completes all electronic documentation in a timely manner, following appropriate protocol. Ensures that the documentation contains pertinent information regarding patient's condition, and appropriate focus of treatment. Uses appropriate agency forms, using approved abbreviations.
  • Case Manages patients who are GIP level of care which includes assuring the patient receives a daily Skilled Nursing Visit (SNV). The SNV will include: an appropriately times (as determined by the Management team) physical assessment through observation, palpation and auscultation, including medical stability, symptoms, including accurately assessing patient’s physical, psycho-social, ethnic, cultural, spiritual needs and patient’s safety needs (e.g. fall risk, abuse and neglect).
  • Documentation of daily SNV will include the symptom being managed, the intervention and the discharge plan.
  • Performs consent visits as scheduled to provide information regarding Provider services and obtains signed consents.
  • Performs evaluation of patients for GIP level of care. Calls MD, Discharge Planner or SW at hospital with disposition as well as liaison report if patient does not meet criteria.
  • Mentors other RN’s to the hospital and GIP guidelines.
  • Assists with community admissions when there are no referrals or GIP patients at the prospective hospitals.
  • Uses appropriate resources (pharmacy, drug manuals) to ensure his/her understanding of actions, interactions, side effects and proper administration of medication. Recognizes and reports adverse drug reactions per policy and procedure. Recognizes and reports medication variances per policy and procedure.
  • Serves as a marketing presence for Provider.
  • Must attend 80% of IDT meetings for the appropriate team.
  • Must attend 80% of Access Team Meetings.
  • Must do statistical work as required by each hospital.
  • Other duties as assigned by Clinical Manager, Access Services.

Qualifications:

  • We're excited to train RN's that are passionate about hospice care
  • Possess a current Maryland nursing (RN) license and 2 years of med/surg experience or home health required. Hospice experience preferred.
  • Hospice certification (CHP) preferred.
  • Demonstrated ability to provide quality patient care. Possess ability to assess patient’s needs, e.g., physical, teaching, mental and spiritual.  Must have strong clinical skills.
  • Must be able to perform the essential functions of the position with or without reasonable accommodation as defined by HR.
  • Ability to recognize safety issues to self and others and be able to assess and implement use of lifts or other personnel to lift and move patients of all sizes and weights in a safe manner.
  • Must have hospice experience and a good working knowledge of Medicare criteria for inpatient level of care.
  • Ability to perform consistently in stressful situations.
  • Basic computer skills, to include but not limited to Microsoft Word and Outlook, required.
  • Strong and creative, problem-solving skills with the ability to prioritize and delegate.
  • Demonstrated commitment to quality, timely, and cost-effective patient care.
  • Ability to simultaneously handle multiple assignments and projects with attention to details, efficiency and accuracy and deadlines.

Schedule:

  • Monday – Friday; 9a-5:30p OR 10a-6:30p (NO weekends or On-Call)

Compensation:

  • $41.80-$45 per/hour
  • $5K sign-on bonus

Work Location:

  • Will split time between set locations in Clinton and the surrounding area

Productivity Requirements:
