The Home Health Registered Nurses (RN) overall responsibility is to provide direct client care and case management to an assigned caseload in the home setting in accordance with physician’s orders. This includes admission of clients for service and determining and developing a plan of care. The Home Health Registered Nurse (RN) assures completeness, timeliness and appropriateness of the patient care plan, its implementation and ongoing evaluation. Assures communication with the multidisciplinary team, support services and community resources to ensure that care goals are achieved in the most efficient and cost-effective manner possible. Ensures that documentation is timely, accurate, complete, and reimbursable.
Qualifications:
- Home Health experience (preferred)
- 2 years of med/surg nursing experience if no previous Home Health experience (required)
- Licensure: State of New Hampshire or Compact State Nursing License
- Minimum ADN preparation
- NH Driver’s License and current automobile and liability insurance
- Tech savvy (preferred)
- Strong documentation, communication, and symptom management skills (required)
- Must be flexible and family oriented
- Current BLS
Schedule:
- Monday -Friday; 8a-4:30p (37.5 hour per week)
- Occasional weekend and on-call requirements
Compensation:
- $34 -$40 Per/Hour
- On-call and Call-back compensated
- Additional incentive compensation 2 x per year (up to $8K)
- Mileage reimbursement at IRS rate
Work Location:
- Out in the field providing patient visits (home)
Coverage Area:
- 30-mile radius of Meredith
