Job Brief: We are seeking a reliable and compassionate Personal Care Assistant (PCA) to provide in-home care services for clients in accordance with service authorizations. The PCA will be responsible for ensuring prompt and uninterrupted delivery of service, emergency and disaster preparedness, and maintaining administrative and professional duties. The successful candidate will be certified and/or licensed in accordance with applicable state regulations
Responsibilities:
Provide personal care services in accordance with service authorizations and ensure that the level, amount, frequency, and duration of services are appropriate to meet the client's needs.
Establish and maintain a 24/7 system for emergency replacement of personal care aides, including a holiday coverage plan for the provision of services.
Maintain procedures to ensure uninterrupted service in accordance with service authorizations.
Share with MCDSS the plan for emergency and disaster preparedness completed in accordance with applicable state regulations.
Notify the client and MCDSS when unable to provide continuing services in accordance with service authorization and make necessary arrangements to ensure client safety.
Notify MCDSS when personal care services appear to be no longer appropriate and do not terminate services to a client without prior approval from MCDSS.
Establish and maintain wages and benefits for Home Health Aides.
Establish and maintain a Home Health Aide Training Program that meets the NY State Department of Health requirements.
Notify MCDSS when accepting or rejecting a patient.
Conduct Nurse supervisory visits every 90 days to evaluate the skill and performance of the person providing personal care services, ensure that personal care services are provided in accordance with the authorization, and prepare written reports for each orientation and supervisory visit.
Provide a replacement promptly when the assigned personal care aide is unavailable, does not work effectively with the patient or caregivers, or is not performing to the satisfaction of the client.
Check time cards, maintain scheduling records and other necessary administrative records.
Skills Required:
Must be certified and/or licensed in accordance with applicable state regulations.
Prior experience providing personal care services is preferred.
Ability to communicate effectively with clients and their caregivers.
Strong attention to detail and organizational skills.
Ability to work independently and as part of a team.
