Job Description
Lara Health is currently in search of a (preferably) bilingual (English/Spanish) Care Coordinator/Case Manager for our Chronic Care Management program, to join our lovely care team and provide patients with care virtually. This is a full time position (min 30 hours a week). Must be US territory based.
COMPENSATION: $20-$25/hour based on experience, contract
THIS IS A FULLY REMOTE POSITION THAT REQUIRES MIN 5 YEAR EXPERIENCE IN COMPLETING ASSESSMENTS AND CREATING CARE PLANS FOR PATIENTS WITH CHRONIC CONDITIONS (PREFFERABLY IN PRIMARY CARE/DIABETES/CARDIOLOGY/GERIATRICS)
Join us for the chance to be part of an amazing team and a great place to work! The Care Coordinator assists providers with ongoing patient monitoring and chronic care management, with the following duties:
● Assisting with helping patients schedule and plan MD visits etc.
● Communicate with patients on regular basis, helping them make lifestyle changes in order to improve their long term health
● Maintain patient electronic medical record (EMR)
● Other duties as assigned
Specific skills and experience required:
● Passion for working with (older) patients, customer service skills
● Min 5 years experience in Care Management
● Preventative patient care background preferred, experience in primary care, cardiology or diabetes clinic highly desired
● Work independently and with a team
● Organizational and time management skills
● Computer skills appropriate to position, you will need your own computer and a reliable internet service
● Written and verbal communication skills, administrative and clerical skills, critical thinking
● Medical terminology use and understanding
Education:
● Associate’s Degree in Human Services or Case Management
● Preferred Bachelor’s Degree in Human Services, Case Management, Social Work, Sociology, and/or Psychology.

This position would be ideal for someone looking to work intensively with patients and who is well organized and able to commit to a min 30 hours a week. You must be bubbly and well spoken as 90% of the work is speaking with pts on the phone.
The role is contract-based and doesn't offer benefits. Some flexibility in regards to working hours (Mon-Fri)
