We are working with a client in need of RN's to work in their facility in Hancock, MI

The client has days, evenings and night shifts available and they are offering a sign on bonus up to $12,500.00 for qualified candidates. Candidates must have an active RN license in MI and graduated from an accredited RN school.

 

POSITION SUMMARY

 RN’s, regardless of specialty or work setting, treat and educate patients and the public about various medical conditions, and provide advice and emotional support to patients; family  members. Nurses record patients’ medical histories and symptoms, help perform diagnostic tests and analyze results, operate medical machinery, administer treatment and medications, and help with cases follow up and rehabilitation.

The Nurse utilizes the nursing process, critical thinking and problem solving skills to provide holistic care to individuals, families, groups, communities and populations across the life span.

AS A NURSE YOU WILL PROVIDE

ASSESSMENT

Provides initial and ongoing assessment and interpretation of patients or clients within the framework of holistic professional nursing practice, and in accordance with the principles of patient and family centered care.  Assesses and interprets information and data regarding the patient or client’s culture and values system, lifestyle, and interest and ability to participate in their care, and other factors impacting successful treatment outcomes.  Assesses and interprets patient or client’s ability (physical, social and psychological) to manage their health in order to transition their care or discharge. In collaboration with the patient or family identifies patient or family needs based on analysis of data.  Identifies and communicates patient or family needs to the interdisciplinary health care team and other providers of care as appropriate.  Provides ongoing monitoring of patient or client’s or family response to intervention; identifies deviations from expected outcomes and assesses change in patient status.   

PLANNING

Develops, coordinates and updates an individualized plan of care, based on evidence, in collaboration with the patient or client and their family, the interdisciplinary health care team and other providers of care promoting continuity of care across the continuum. Prioritizes nursing interventions to achieve patient or client outcomes and goals.  Develops and coordinates a comprehensive discharge and education plan to meet the patient or client and their families’ needs, including health promotion, self-management, identifying and coordinating resources to promote continuity of care across the continuum.

INTERVENTION

Establishes and maintains a therapeutic relationship with patient or families. Identifies, initiates and coordinates therapeutic interventions consistent with the legislated scope of practice and level of competence. Implements prescribed interventions (e.g. diagnostic tests, medications) and performs independent nursing interventions (e.g. patient or client and family education).  Ensures the implementation, coordination and ongoing evaluation of the patient  or  client care plan  Provides counseling that assists the patient or client in developing, improving, or regaining skills, social and community supports, promoting health and recovering from illness and disability.  Consults with the interdisciplinary team and other care providers to discuss changes related to patient or client status, and modifies interventions.  Interprets medical information including diagnosis and treatment plans to patient or client and responds to patient or client questions and concerns.  Provides information and instruction to assist patients or clients to understand health care practices and processes.  Utilizes educational material to support patients or clients understanding of their health care, health promotion, and or health maintenance, and the enhancement of their individual well-being.  Liaises with other providers in the community to transition care. 

EVALUATION

Utilizes data obtained from the patient or client and their family and other health care providers to evaluate the plan of care including the patient or client’s responses to interventions and achievement of expected outcomes.  Supervises and evaluates care provided by direct care assistive and support staff.  Uses data and evidence based on best practice to evaluate nursing activities and interventions.  Documents complete accurate and relevant data for every patient or client according to facility policies.

ADVOCACY

Provides a supportive environment for communications and decision making by patients or clients and their families.  Advocates for and supports the principles of patient and family centered care, and the patient or client’s rights and well-being.  Assists patient or client and families, to become aware of issues relevant to their health care needs and provides information on resources and services within the health care system.  Recognizes trends in health care needs within patient or client groups served, and involves the patient or client in planning mutually expected outcomes where possible and appropriate.  Demonstrates knowledge and understanding of legislation dealing with patient or clients’ rights and responsibilities in health care.  Demonstrates knowledge of the roles, responsibilities and rights of other nurses and other health care professionals.
